Sometimes it is useful to see a graphical picture of
your medical history, correlating in time the history
of your PSA and other data with your tests and treatments.
The medical information is on one-page summary of PSA and any other numerical data you may have, other test results, and treatment starts and stops.
There are two ways of getting your Medical History Graph:
1. Medical Smart Chart - Enter your data online by clicking here.You will be able to enter your information using drop down menus. With a click of an icon, you will be able to get your one page MediGraph instantly. This can be repeated on an ongoing basis. You will have the ability to edit and change your own record. You will be able to print or download the your chart.For assistance write to Info@pcref.org

2. Multigraph - Input your information on line here. or Create a chronological digest file and email it to Multigraph@pcref.org. Click here for detailed instructions.
Medical History charting of your data is a service provided at no charge by the Prostate Cancer Research and Education Foundation, San Diego.
