Graph Your Medical Data

A fundraising service of the Prostate Cancer Research and Education Foundation. While no payment is required,we hope to encourage you to have a reason to make a donation to PC-REF.
It is useful to see a graphical picture of your medical history on a single page,showing the time relationship of your treatments and procedures on your PSA and other test results. The trend of your PSA is known to be particularly significant. These charts can be helpful to your physician when reviewing your case. We encourage you to take your one page medical history graph to your next office visit.

Medical History Graphs can be simple or complicated.Click on the thumbnails to see sample graphs.

 


PC-REF offers two distinct graphing services,Medical Smart Chart(MSC)(automated) and MultiGraph(manual).

With MedicalSmartChart,you enter your data using drop menus online and you see your graph instantly. With the MultiGraph service, you fill out a form on line and send it by email to MultiGraph@pcref.org  You will receive your graphs by email within a few days, along with a data file that you update as you get more information.
With both systems, you have control over your personal medical information. With Medical Smart Chart,you can give permission to your physician to log in with your password and access your chart in his office or in the Emergency Room. With MultiGraph,you can specify to whom your results are emailed.

How you get your Medical Smart Chart

Click on  MEDICALSMARTCHART- online you will enter your information using drop down menus.  With a click on View Graph Menu, you will be able to receive your one page Medical Smart Chart display instantly.  Also,when you click on “View and Print Data Reports”, you will generate two reports, a chronological report,and a comprehensive table with all the tests grouped together.  This can be repeated on an ongoing basis.  You will have the ability to edit and change your own record instantly.  You will be able to print or download your Medical History.  You also can receive reminders to update your MSC(Medical Smart Chart)at intervals that you can specify. For assistance write to SmartChart@pcref.org

How you get your MultiGraph

To enter your data online the first time,click on MultiGraph.Or,if you would like to send in a data file, click here for instructions. With either method you will receive a number of graphs (depending on your data), a data file to update, and additional instructions. If you have any questions send email to MultiGraph@pcref.org.

Medical History Charting of your data is a service provided at no charge by the Prostate Cancer Research and Education Foundation,SanDiego. However,your donations to PC-REF are greatly needed and gratefully accepted.  Please remember that our mission,to provide seed money for new research in prostate cancer,is your mission too!
